These questionnaires are used by our underwriters to evaluate your submission.
Please submit the appropriate questionnaires with your Acord form. Questionnaire
1-Nonprofit General Profile provides us with general information about
your submission that is not included on the Acord form and will help you
determine which additional questionnaires to complete.
These questionnaires can be completed by hand or on the computer by opening the documents in Adobe Acrobat and choosing the Text Select Tool.
If you are submitting the completed questionnaires with an Acord form, please send them together by the same transmittal method. If you are submitting the completed questionnaires separately and know your underwriter’s name, you may submit them directly. If you do not know your underwriter’s name or one has not been assigned to your agency yet, please submit the completed questionnaires by mail, fax or e-mail to:
